Reservations

We appreciate receiving as much notice as possible when you need to use our service. You can make a reservation by calling our office at (505) 332-1919, by sending an email to , or by completing the online form below.

In order to use our service, you must provide us with a signed Client Service Agreement and a completed Client Assessment Information sheet. If you will be accompanied on your trip, we also need a signed Ride-Along Companion Agreement.

For your convenience, we have posted copies of all three of these documents on our site so that you may print them and send them to us in advance our your first reservation time.

Once completed, you may fax the forms to us at (505) 332-0190, mail them to Go-Fors Inc. TOO, PO Box 94324, Albuquerque, NM 87199, or email scanned copies of the forms to .
